I have filled in this form several time and keep telling me ,my credentials are not found or words to that effect,each time I double check but no good credentials not found grrrr

Try leaving if you have any illnesses blank - that worked for me.

But I just said no

Leave it completely blank - don’t even put yok in - it worked for me
