. UK Locator Form including code for 2 and 8 day UK PCR tests
. Proof of payment for 2 and 8 day UK PCR tests

It depends on where you were vaccinated - If vaccinated in TRNC the answer is YES, if vaccinated in UK or most other countries in the world the answer is NO.
The rules are published here, currently you have to follow the unvaccinated rules (scroll down in link) if you have been vaccinated in TRNC
