UK to Ercan, locator form?

Can anyone please tell if you need to complete a locator form when travelling from the UK via Istanbul to Ercan?

You need the ADA pass for the TRNC but not for Turkey as you are transiting.

It will ask you to download your vaccination record but the app never worked so as many have had the same issue and stated just proceed it still issues the certificate but you would have to prove by paper certificate on request.
